What do I need to bring to my first office visit?
Please bring the following to your first visit:
Patient medical history form
Patient questionnaire form
List of previous surgeries and medical problems
List of current medication and allergies you may have to medication
Name and address of referring and family physician
Insurance information and referral (if applicable)
All x-rays, scans, and pertinent test results

What healthcare plans do you accepts?
We accept the following insurance plans: Blue Cross Blue Shield Principle, Aetna US Healthcare, Cigna, Amerihealth, Medicare, Mid Atlantic Health Systems, United Health Care

If my doctor is out and I have a problem, can I still get assistance?
You can call 277-8787 after hours for emergencies only.

What will I be expected to pay at the time of my visit?
The amount that you will be expected to pay at the time of your visit depends on your individual insurance and circumstances. Patients with valid HMO or PPO referrals are expected to pay their office visit co-pay.

Is your office easily accessible by wheelchair?
Yes. There is wheelchair access to the front of the building.

Do you treat out of network patients? Yes. We are happy to see patients who are out of our network. However, the payment for the cost of the visit will be expected at the time of service.
